Could happen to anyone, but glad to see it is a firm with "lots" of carbon fiber experience. Hah! Their first carbon production model was 2004 or so. They need to crack the whip harder in "their" Chinese factory, or pay more for quality control. The link to the recall notice

Bruce unless they changed they don't use just one factory where they would control production such as FSA. They bid their work out. Scary thought isn't it? I have a friend who wrenched in a Cervelo Store...he would not buy one no way no how.

Your right, it is even worse. I just hate these companies that say we have to do carbon, so let's just outsource it. Trek, Calfee, Look and a handful of others are the real leaders in carbon. Everyone else (Cannondale, Cervelo, etc.) are simply copy cats. If metal is such a wonderful frame material, then keep building with it and shut up about carbon. But they know carbon is the future and they are scrambling to keep up.
People seem to buy their bikes though. The power of marketing.
