Pedal Failure - w/ serious injury

I am looking for information on pedal failure. I would like to know if anyone has had a pedal failure that resulted in injury. This injury required 7 hours of surgery to repair.

I was using LOOK PP357 pedals for about 8 months when the spindle (axle) failed. It sheared off just at the inboard bearing.
