A qualified yes to both questions. Most likely a maintenance issue however, but could also be in your shifters. Tiagra shifters aren't quite as smooth shifting as Ultegra or Dura Ace.
All that said, I would tell you to do a complete tune-up on your running gear
first
: tune and service both front and rear dérailleurs, check chain for wear and replace if necessary and shoot a little lube into both shifters.
